Why Most Private Practices Struggle to Hire the Right Clinician

Hiring a clinician sounds simple.

Post a job.

Review resumes.

Schedule interviews.

Make an offer.

But in reality? It’s rarely that straightforward.

We’ve supported many growing practices, and the same hiring challenges tend to show up again and again, especially when owners are already stretched thin.

Here are a few of the most common reasons private practices struggle to hire well.

1. The Job Description Is Too Broad

“Seeking licensed therapist.”

That’s often where it starts...and where things go wrong.

Without clarity around:

  • Ideal population

  • Specialty areas

  • Compensation structure

  • Caseload expectations

  • In-person vs. hybrid vs. remote

  • Culture and values

You’ll attract a wide pool… but not necessarily the right one.

Clarity upfront saves significant time on the backend.

2. There’s No Structured Screening Process

A strong resume doesn’t automatically mean a strong clinical or cultural fit.

In group practices, especially, culture alignment matters deeply. How someone communicates, manages documentation, responds to feedback, and supports shared values can make or break the team dynamic.

Without a structured screening process, it’s easy to:

  • Interview inconsistently

  • Overlook red flags

  • Make decisions based on urgency instead of alignment

3. Follow-Up Is Too Slow

Strong clinicians often have multiple options.

When hiring is squeezed between sessions, supervision, and admin time, follow-up can unintentionally lag. Even a few days can mean losing a great candidate.

Recruiting requires consistency and momentum, something that’s hard to maintain when you’re also running a full caseload.

4. Hiring Happens From Overwhelm Instead of Strategy

This is the big one.

A full waitlist feels exciting AND stressful.

When you’re maxed out, the instinct is often:

“I just need help.”

But hiring works best when it’s intentional:

  • You understand your break-even numbers.

  • You know what your onboarding process looks like.

  • You’re clear on what success will mean for this role.

Growth shouldn’t feel reactive. It should feel steady.

Building the Right Team Takes Structure

Hiring the right clinician isn’t just about filling a seat.

It’s about:

  • Protecting your practice culture

  • Supporting client care

  • Sustaining your own energy

  • Building something that lasts

With the right structure and processes in place, hiring can feel thoughtful rather than chaotic and aligned rather than rushed.

If you’re at the point where growth is knocking on your door, it might not be about “posting a job.”

It might be about building a system that helps you hire well.
